A Life Care Plan or Cost of Future Care assessment is a comprehensive, evidence-based evaluation that outlines the current and future needs of an individual following injury, illness, or trauma. As a Canadian Certified Life Care Planner (CCLCP), I develop detailed, defensible plans that integrate medical, rehabilitative, psychological, vocational, and functional information to project long-term requirements such as treatment, therapies, equipment, attendant care, housekeeping and home maintenance services, home modifications, and community supports. This process involves systematic analysis, consultation with treatment providers, and adherence to established life-care planning methodologies to ensure accurate forecasting and transparent rationale. The life care plan provides objective, structured recommendations that assist in rehabilitation planning, legal contexts, insurance matters, and long-term resource allocation.
